Ontario Soccer Forms
The below forms are used to make players eligible to play for other teams in circumstances that are extraordinary to an internal call-up. All of these forms must be submitted to Guelph Soccer and approved by SWRSA before they are valid. We strongly encourage our coaches to utilize available internal call-up channels before considering players that require the below forms.
Temporary Eligibility Permit - to make a player registered currently to a club other than Guelph Soccer eligible to play for Guelph Soccer on a temporary basis
Trial Registration Permit - to make a player NOT registered to ANY Ontario Soccer-sanctioned club eligible to play for Guelph Soccer on a temporary basis for specific games (do not use for tournaments - use the below form)
Short Term Registration Permit - to make a player NOT registered to ANY Ontario Soccer-sanctioned club eligible to play for Guelph Soccer on a temporary basis for a specific event or tournament
